This Contact form is similar to our AFC Easy Contact Form in that it will send the owner the persons name, email, phone number, referred by, contact type and a question / suggestion or feedback. The form includes email validation and will goto an error page should the form not be filled out properly. If the form is correctly filled out it will goto a Thank You page. The main difference is that the AFC MySQL Contact Form will send an eMail Confirmation to the client and will store all the data into an MySQL database, plus it will provide you with a Table Display so you can view all the data that has been entered at anytime. The script includes instructions and is completely modifiable to suit your needs. There are no restriction and the code is open source. So feel free to add to it or change it as you please.
